Filing for Divorce or Dissolution

Please ensure you have all of the documents listed for the type of case you are filing. Documents may be printed from the forms pages of the websites for the Supreme Court of Ohio and the Franklin County Domestic and Juvenile Court.

You must answer all questions and complete all blanks on all documents, otherwise the Court may not be able to make orders in your case. Do not sign any documents until they are notarized. Documents may be notarized in advance or at our office with valid photo ID.

Divorce With No Children

The following documents can be found on the forms page of the Supreme Court of Ohio website:

  • Complaint for Divorce
  • Judgment for Divorce without Children
    Needed for final hearing. E-File as "Filing Submitted to Judge" in Word (.doc) format.

The following documents can be found on the forms page of the Franklin County Domestic and Juvenile Court website:

  • Confidential Disclosure of Personal Identifiers
  • Affidavit 1 (Affidavit of Income and Expenses)
  • Affidavit 2 (Affidavit of Property)
  • Standard Mutual Temporary Restraining Order

Additionally, you will need:

  • Certificate of Assignment(PDF, 193KB)
  • Filing fee of $275 plus Service Fee (see Service Options below)
  • Service Options
    • Certified Mail: $10.00
    • Sheriff Service: $30.00
    • Foreign Sheriff (any non-Franklin County Sheriff): $75.00
    • Process Server: $4.00
    • Registered Mail: $27.00
    • Waiver of Service: No fee
